Responsibilities 1.
Assists in the identification of member populations needing navigation services and carecoordination including obtaining admission, Discharges and Transfers from Health SystemAcute facilities and generating Risk-Stratification and Predictive Modeling reports.
2.
Effective collaboration, communication and coordination among all responsible parties of anindividual members multidisciplinary health care team striving to eliminate fragmentation,duplication or gaps in health care.
3.
Identifies potential barriers related to patient's home setting and self-care/management needsand refers patient to resources to help resolve issues.
4.
Assist as a liaison with members and their families to physicians, clinical staff, communityresources and others.
5.
Actively participates in quality activities by communicating and coordinating with thenavigation team in the development of tools for optimal consumer outcomes and reportfindings.
6.
Proactively acts as a members advocate, responding to and working to resolve concerns.
7.
Researches member eligibility and assigned Population Health Value-Based Program throughchosen analytical tool.
8.
Abstracts meaningful data from the EHR/clinic medical record of identified high-riskconsumers.
Qualifications Education Required:
BSN Experience 2years of cardiac nursing experience Skills Knowledge base regarding cardiovascular care and specialty area of navigation with education tosupport that knowledge base to include community and cardiac program assessment, resolution system barriers, the cardiac continuum, health disparities and cultural sensitivity Licensure/Certifications Required:
Current RN license Recommended Skills Cardiac Nursing Clinical Works Coordinating Diversity Awareness Knowledge Base Medical Records Estimated Salary: $20 to $28 per hour based on qualifications.
