This is a Bargaining-Unit position The incumbent provides program coordination for the Veterans Integration to Academic Leadership (VITAL) program. Provides supported education services and counseling to Veterans enrolled in local colleges and universities. The incumbent is also responsible for providing outreach tasks and helping Veterans who have clinical needs to enroll in needed healthcare. This task is done in collaboration with the Transition Care Management Team. The incumbent is embedded at the University and has dedicated office space on site. Incumbent also spends time in the community and at the local VAMC based on needs. The VITAL Program Coordinator provides leadership to a multi-disciplinary team working primarily in the community with Veterans who are attending local colleges and universities. The incumbent practices independently, providing clinical supervision and administration, as well as psychosocial and case management services at an advanced practice level to eligible Veterans and their family members to meet biological, psychological, social, and environmental needs. Veterans who are served come from the Texas A&M in College Station and potentially more colleges and universities in the future. Their health care and psychosocial problems and needs are complex and require a high degree of clinical oversight and creative problem-solving. Services are provided in community settings. The incumbent also works with university and college faculty, staff, and administrators in order to advise the university on how to effectively serve their Veteran populations. The incumbent also provides NIC programs with high quality case management, clinical treatment, advocacy, and to coordinate linkage with appropriate VA and community service providers / agencies as needed by the client. This is accomplished in collaboration with other members of the interdisciplinary treatment teams as appropriate i.e. Specialty Clinics, NHCU etc. The incumbent develops and carries out outreach strategies and activities at local colleges and universities and at times within the community. This task is done in collaboration with CTVHCS' TCM team and university and college faculty, staff, and administrators. The incumbent collaborates with Student Veterans and Student Veteran Organizations to enhance outreach effectiveness while empowering Veterans. The incumbent also works individually with Veterans enrolled in these schools who have unmet clinical needs to help those Veterans enter needed care at either a VA or other appropriate facility. The incumbent will also have knowledge of community resources and how to make appropriate referrals to community and other governmental agencies for service and ability to coordinate services. The incumbent will also provide a program of outreach to identify and locate Veterans in need of home care. Outreach is to include communication and coordination with this Medical Center, other governmental and private community health and social service agencies. United States Citizenship:

Must have a Master's degree in Social Work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE) (transcript required). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work. Licensure:
Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the advanced practice level. Current state requirements may be found on the OHRM website. English Language:
Must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, Chapter 3, section A, paragraph 3j. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation). Experience/Education:
The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior Social Workers are experts in their specialized area of practice. Senior Social Workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs:
1. Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. 2. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. 3. Ability to supervise multidisciplinary staff assigned to the program. 4. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). 5. Ability to provide training, orientation, and guidance within clinical practice. The full performance level of this vacancy is GS-12. Physical Requirements:
